Change Personal Data
If you change your name, either as a result of a marriage, divorce, etc., or you change your address, you must notify Human Resources. Be sure to complete the Change of Personal Data form, and include any supporting documentation that may be necessary. You also may need to fill out forms to change your name and/or address with your respective health insurance plans. Not having an updated name and address on file in Human Resources can result in mail not being received at your new address, health claims being rejected due to a new name that has not been registered with the proper plan administrators, and an incorrect W-2 being filed with the IRS.
