New Student Deposit
The New Student Deposit Form should be submitted by accepted students to the Office of Undergraduate Admissions within 21 days of the date on the acceptance letter. Accepted students wishing to attend the university should submit this completed form with the $200.00 new student deposit.

Reactivation
The Reactivation Form should be used by an applicant wishing to apply to UMass Boston who has already submitted an application to the Office of Undergraduate Admissions within the last year. This form, the $60 non-refundable fee, and all application material (including any new transcripts) must be submitted by the appropriate posted application deadline to the Office of Undergraduate Admissions. If we are not able to reactivate your application an Admissions Counselor will contact you.
Change of Major
The Change of Major Request Form can be used by applicants wishing to have their application reviewed for a different major or by accepted students wishing to adjust their major before the end of their initial add/drop period. Submitting this request to the Office of Undergraduate Admissions does not guarantee admission into the selected major. Any change of major requests by students after the add/drop period should be filed with the One Stop in the Campus Center.
Deferral
The Deferral Form is to be used by accepted students wishing to postpone their acceptance to a future semester. Students are allowed to defer for a maximum of one year from their original acceptance date. A student may not defer if attending another college/university or s/he has been accepted by the College of Nursing & Health Sciences Bachelor of Science in Nursing program. This form and its $25.00 non-refundable fee should be submitted to the Office of Undergraduate Admissions by the deadline stated on the form.
Transfer Verification for Non-Immigrant Students
The Transfer Verification Form for Non-Immigrant Students should be submitted if you are an admitted non-immigrant international student on a current F-1 visa transferring from another U.S. college, high school, or language academy.
