Questionnaire Instructions

University of Massachusetts Boston Performance Review Provost and Senior Vice Chancellor for Academic Affairs

As called for in the Trustee policy, University Guidelines on the Review and Evaluation of Senior Administrators (Doc. T93-080) it is now time for the five year performance evaluation of the provost and senior vice chancellor for academic affairs at the University.  As the chief academic officer of the University, the provost has a wide ranging impact on the campus.  The provost’s role is one that is critical to the future of the University and one whose vision can advance the opportunities the University offers to students, faculty, members of the research institutes/centers, academic staff and the various communities it serves or with which it interacts.

As detailed in the University guidelines referenced above, the chancellor performs the actual evaluation of the provost: the review committee is consultative to the chancellor.  This process is not a re-appointment process.  The purpose is to evaluate the performance of the provost with regard to:

  • leadership
  • policy development
  • management and administration
  • personnel
  • affirmative action/diversity
  • contributions to shaping the intellectual life of the campus

The Review Committee is committed to having an inclusive and comprehensive process and is seeking input from faculty, staff, and students of the University through a survey that can be accessed online.  We hope that you will take the time to share with us your considerations on the performance of Provost Fonteyn and to provide some guidance to the committee as to the areas for affirmation as well as those for improvement.  Your assessment of his performance is extremely important to the University and to the continuing development of the Office of Academic Affairs.  We ask your candid assessment of Dr. Fonteyn’s performance using the attached questionnaire.  Responses will be confidential and anonymity will be maintained by the committee.  No response will be linked directly or indirectly to any individual at any time in this process. 

We would ask you to fill in the demographic information on this questionnaire and again assure you that no information that you provide will be linked directly or indirectly to you individually. In doing the ratings, if you feel that you lack the information to respond to an individual question please check the box that says “cannot rate”.  Space is provided for you to make comments.  In completing the narrative please provide the committee with observations of the provost that you feel should be reaffirming of his performance as well as areas where there may be an opportunity for improvement.  We have structured the questionnaire so that it will provide us with significant information and input while not presenting a significant drain on your limited time.

If however, you feel that you would like to submit a written letter in lieu of, or in addition to, completing the questionnaire please feel free to do so and then follow the same procedures for submitting your comments (either letter or questionnaire). 

You must complete and sign your cover sheet (found at this web-site) when you submit a survey and/or letter. When they are received, cover sheets will be separated from surveys and letters before they are read and compiled.  Thank you for your participation in this important review.
