Urban Connections Survey
Are you on the map?
UMass Boston is developing a Community Resource Center to serve as a clearinghouse for information about connections between the University and its many communities. As part of this effort, the Chancellor’s office is producing a map that will show where and how we connect. The first version of the map will make its public debut at the October 22, 2004 “Urban Connections Forum.”
To have your research, teaching, or university-related service activities documented on the map, please complete our brief questionnaire. (Note: if you work with more than one community or community group, please make and fill out multiple copies!).
Download the Survey:
- Urban Connections Survey in Microsoft Word format (RTF file)
- Urban Connections Survey in Adobe PDF format (PDF file)
You will receive a follow-up call that will allow you to verify and expand upon the information you supply. Thank you for putting yourself and UMass Boston on the map! Note: Please list any interactions or projects that were active from January 2003 onwards even if the project has since concluded.
Please complete this form as soon as possible and return it by campus mail to: Molly Flavin, Office of Service Learning and Community Outreach, UMass Boston.
