STEP 12: Prepare Appointment Forms



From this point on, another procedure will take over. You need to inform the administrator in your Dean/Director's office to prepare or oversee the preparation of appointment forms, which consist of both the pre-employment and the employment paperwork.

The pre-employment paperwork includes those forms/documents that every new employee must complete prior to being placed on the University payroll.The forms include:

  1. Personal Data Questionnaire,
  2. State and Federal tax forms,
  3. TB Test,
  4. I-9,
  5. OBRA Deferred Compensation Form (for non-benefitted employees), and
  6. Direct Deposit (optional).

Currently, the documents each new employee needs to receive include:

  1. Guide to Conflict of Interest Law,
  2. Guide to Political Activity,
  3. Sexual Harassment Policy,
  4. Drug-Free Workplace Policy, and
  5. University of Massachusetts Principles of Employee Conduct.

Please note that employees are not required to provide letters of reference to be placed on the University payroll, but letters of reference may be required as part of the recruitment process (as in the case of most staff and all faculty positions) prior to making an appointment. We strongly encourage you to ask for the names of references, or letters of reference, to help you identify the best candidate as you go through your recruitment efforts. Likewise, letters of reference can be forwarded to Human Resources for placement in the new employee's personnel file.

The employment paperwork includes:

  1. Personnel Action Forms, and/or
  2. Offer and Acceptance Forms for Appointment or Rehire.
