Please note that this form is for reserving audio/visual equipment. A Student Computer/Projector Reservation Request must be submitted to reserve that equipment.
Please print or type the above information. Incomplete forms may delay your reservation.
No equipment will be loaned without a current student ID.
Equipment is available for use on campus during our normal operating hours.
