Financial Conflict of Interest

Your Responsibilities

To begin, please read the University of Massachusetts Policy for Promoting Objectivity in Biomedical Research.

All investigators involved in sponsored research are required to complete and submit a financial conflict of interest disclosure form to the UMass Boston Office of Research and Sponsored Programs (ORSP) prior to the submission of an application for any form of sponsored funds.

Submitting an Application to a Public Health Service Agency

Examples of designated Public Health Service (PHS) agencies include, but are not limited to, the National Institutes of Health, the Agency for Healthcare Research and Quality, and many others. For a complete list of PHS agencies, please see the U.S. Department of Health and Human Services Organizational Chart.

The PHS financial conflict of interest regulations have unique reporting and conflict management requirements. PHS regulations require an initial disclosure to the UMass Boston ORSP of all financial interest related to the institutional responsibilities of the investigator. For guidance about the PHS regulations, refer to the National Institutes of Health Policy on Financial Conflict of Interest.

  1. Now, please download to your computer and complete the form UMass Boston Summary Disclosure of Financial Interests Related to Projects Sponsored by Public Health Service (PHS) agencies including the National Institutes of HealthNote: The lead principal investigator on a proposed PHS application for funding is responsible for obtaining a PHS disclosure form as well for all participating investigators. For the purposes of the PHS disclosure, “Investigator” means the project director or principal investigator, co-investigator, postdoctoral associates, students, and any other person, regardless of title or position, regardless of whether paid or unpaid, who is responsible for the design, conduct, or reporting of research funded by the PHS, or proposed for such funding, which may include, for example, consultants, or subrecipients (e.g., subawardee or consortium member).
  2. If you checked the “No” box, sign and date the form, and then clearly print your name and title.
  3. If you checked the “Yes” box, sign and date the form, clearly print your name and title, and then turn to page 3 and complete the Investigator’s Detailed Disclosure Form.
  4. Mail or deliver your completed disclosure form to your preaward grant administrator in the Office of Research and Sponsored Programs, Quinn Administration Building, 2nd floor.

Submitting an Application to a Non-Public Health Service Agency

For all non- PHS submissions, follow these steps.

  1. Download to your computer and complete the form University of Massachusetts Disclosure Summary for Investigators Proposing or Conducting Federal Research (Non-PHS-Funded)Note: The lead principal investigator on a proposed non-PHS submission for funding is responsible for obtaining a PHS disclosure form as well for all participating investigators.
  2. If you are submitting a proposal to the National Science Foundation (NSF), then read and complete the section “NSF Proposals”. For all other non-PHS submissions, please read and complete the section “Other Federal Proposals (non-PHS, non-NSF).
  3. If you checked the “No” box, sign and date the form, and then clearly print your name and title.
  4. If you checked the “Yes” box, sign and date the form, clearly print your name and title, and then turn to page 4 and complete the form Investigator’s Detailed Disclosure of Financial Interests Related to non-PHS-Funded Projects.
  5. Mail or deliver your completed disclosure form to your preaward grant administrator in the Office of Research and Sponsored Programs, Quinn Administration Building, 2nd floor.

For Subrecipients

All proposed sub-recipients must demonstrate compliance with the Federal Conflict of Interest regulations by completing and submitting the UMass Boston Subrecipient Statement of Intent form.

For Non-UMass Boston Investigators

Form to be completed for all non-UMass investigators, who are not subrecipients and who are not affiliated with another institution, must complete and submit the Non-UMass Boston Individual Investigator Disclosure of Financial Interests form.
