If you have access to your My Health Beacon Portal and have submitted or received immunizations at UHS, you can access and print your immunization records on the My Health Beacon Portal. 

1. Log in to your My Health Beacon Portal and click on the "Immunizations" tab. 
2. On the top right-hand side, click "Print." This will give you a view of all your immunizations from our portal with dates. 

Whether you want UHS to release your medical information to another provider or if you need copies of your records, you can request your medical and immunization records by following the steps below:

1. Download the Authorization to Release Medical Information form and complete it.
2. You can send it back to us by:
