Graduate Student Forms 

By law we are required to use your legal/primary name to process the following forms:

• Additional Program Form
• En-Route Degree Application (etrieve)
• Graduate Student Full-Time Continuation Form (etrieve)  
  • This form is NOT for students who have Financial Aid. If you need to be full time for Financial Aid reasons complete the Dissertation Thesis form found on the Financial Aid Forms Website
• Leave of Absence Request Form
• Program Change Form 
  • This form is for current graduate students (Masters & PhD) who wish to discontinue their current program and enter another 
• Program Fee Enrollment Form
  • Most students can enroll themselves in Program Fee (CAS 600) via WISER self-service. Please review this help video on how to enroll. Note, the program fee enrollment form is for active students only. Inactive students must complete the RE-Enrollment Application  below. 
• Re-Enrollment Application  
  • Re-Enrollment Application Deadlines:
    >>Fall Semester: August 15
    >>Spring Semester: January 15
• Statute of Limitations Extension
• Graduate Transfer Credit Permission (etrieve)
• Withdrawal Form
  • The withdrawal form is for current active students who wish to withdraw from their program. Please review the University Withdrawal Policy for more information. Newly accepted students please contact graduate.admissions@umb.edu to defer or cancel your acceptance before the start of your first semester.
