Uploading Documentation

Students can upload vaccine records, lab results, and other health records through their My Health Beacon Portal. This is the easiest and fastest method for submitting all records. 

Tuberculosis (TB) Screening

  1. Log in to your My Health Beacon Portal to access the web form.
  2. Click on the "Health Requirements and Forms" tab on the home page, located in the left-hand column. This will bring you to your individualized clearances page, where you can track the status of all your requirements.
  3. Locate the "TB Screen-Risk Form" requirement and click on the "Update" tab to access the TB Screening Form.
  4. Depending on the volume, UHS will work to review your submission as soon as possible and update your status. 

Uploading Vaccine Records, Lab Results, and Other Documents

  1. Log in to your My Health Beacon Portal to upload your documents.
  2. Click on the "Health Requirements and Forms" tab on the home page, located in the left-hand column. This will bring you to your individualized clearances page, where you can track the status of all your requirements.
  3. Locate the "Immunizations Records" requirement and click on the "Update" tab to arrive at the upload window.
  4. Click on the "Upload" tab and select the document you want to upload.
    • Please ensure all documents have your full name and date of birth.
    • Documents submitted without identifiable information will not be accepted.
    • Accepted upload formats include; gif, jpg, png, and pdf. Do not upload MS Word documents.
  5. Depending on the volume, UHS will work to review your submission as soon as possible and update your status. 

Other Methods to Send Health Documents

Health Records can also be faxed and mailed to University Health Services. However, we highly recommend uploading documents through your My Health Beacon Portal to expedite the verification process.

Fax records to 617-287-3977. Attention: Immunization Compliance Coordinator. Please ensure all pages include your full name, date of birth, and UMB ID.
